An unannounced case management visit made out to the facility on 01/07/25 at 10:00 AM, by Licensing Program Analyst Pang Lee who was met by administrator Josephine Saez. The current census is 30.
LPA Lee explained the purpose of this case management visit was to interview a client in regard to a compliant control # 27-AS-20241205110131 that is unrelated to this facility. During the interview Alta Regional Service Coordinator Mai Thor was also present.
Per California Code of Regulations, Title 22, no deficiencies were observed during this visit. Exit interview was conducted and a copy of this report was provided.
