Complaint Investigation Report

Allegation: Facility is not conducting fire drills. LPA conducted interviews and record reviews as stated above. Based on the evidence reviewed, it was revealed that that fire drills have been conducted at the facility with written proof provided to LPA demonstrating consistent drills conducted from January 2026 to current. Fire drills are conducted monthly. As a result, there is not a preponderance of evidence to conclude fire drills are not being conducted, and this allegation is UNFOUNDED. A finding of unfounded means the allegation is false, could not have happened, and/or is without a reasonable basis.

An exit interview was conducted with the Administrator and a copy of this report was provided.
