Facility Evaluation Report

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with Licensee Robert and Co-administrator Jarrett.

LPA Valerio and facility staff Nico conducted a tour of the physical plant. LPA Valerio observed resident bedrooms to be clean, organized, and free from debris. Resident bathrooms were observed to sanitary with hygiene supplies. Common area were clean and organized. The facility was observed to have an adequate food supply. Medications, sharps, and toxins were inaccessible to residents in care. The exterior plant was observed to have an area for outdoor activities and seating. No emergency exits were obstructed. The fire extinguisher and fire alarms appeared to be in working condition. The last fire drill was conducted on 10/05/2025.

LPA Valerio reviewed staff and resident file. All files were up to date with annual documentation. No health or safety concerns observed during the visit

LPA requested the following: LIC 500, LIC 308, Emergency Disaster Plan, Updated Administrator Information, Surety Bond

Per California Code of regulations (CCR) - Title 22 - no deficiencies are being cited. An exit interview was held, and a copy of the report was provided.
