Facility Evaluation Report

Unannounced Annual Inspection visit was made by Licensing Program Analyst (LPA) Kimberly Viarella to this facility on 04/20/26. LPA identified herself to the staff on duty, explained the purpose of the visit, and asked to speak with Designated Facility Administrator. LPA met with facility Designee, Shirley Lewis, and a brief meeting followed.

LPA toured the facility. There were 20 clients and 9 staff present at the time of this inspection. The Designee stated that they generally ran with a ratio of 4:1. Clients were having lunch in the dining area when this LPA arrived. LPA observed the exercise room, computer area, large classroom, storage room, office and staff room. All contained the required furniture and furnishings to be in compliance.

All sharps and cleaning supplies were observed to be locked and inaccessible to participants. The facility temperature thermostat provided a reading of 69*F. LPA observed multiple fire extinguishers in the hallway and a pull alarm system. LPA also observed that the fire extinguishers were last inspected on 10/07/25 by Sacramento Fire Extinguisher Company and were also in compliance.

A first aid kit was inspected and observed to have all of the required elements. The Designee stated that they currently did not have any program participants who required medications, although, when they have, all medications have been stored in the Administrator's locked office and were inaccessible to the client participants. LPA reviewed administration, storage and destruction procedures with the Designee to ensure compliance with Title 22 regulations.

LPA then reviewed the Emergency Disaster Plan and emergency drill log with the Designee. All were in compliance at the time of this inspection.
