Facility Evaluation Report
On 7/8/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct their annual inspection visit. LPA met with a staff on duty, (S2), and stated the purpose of the visit. The Co-Administrator Marilyn de la Cruz (S1) was notified of the visit and arrived shortly after.
Overview : Facility is a two-story home located in a residential neighborhood. Facility is licensed to serve up to 6 adult residents, ages 18 to 59, all ambulatory only. Facility is also vendored by Regional Center. Facility has one bedroom and one bathroom downstairs and the upstairs has two resident bedrooms, a staff room with bathroom and one hallway bathroom for resident use.
Initial Observation: Upon arrival LPA noted 3 staff on duty (S2, S3, S4). S2 stated that residents were out in the community at this time; but returned to the facility later during the visit.
Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA observed the inside of the facility to be clean and in good repair at this time. LPA inspected 3 resident bedrooms and were observed to be equipped with the required furniture and sufficient lighting throughout. LPA measured the hot water temperature in the downstairs bathroom at 112 degrees Fahrenheit. Both resident bathrooms were observed to be in clean and good repair at this time. Room temperature was observed at 75 degrees Fahrenheit.
