Facility Evaluation Report
On 4/17/2026 at 1:00 PM Licensing Program Analyst (LPA) Shakaricka Hughes conducted an unannounced visit to the facility. The purpose of the visit was to conduct a case management visit regarding citations that were issued to the facility on 03/24/2026. LPA spoke over the phone with the facility licensee/administrator Mohamed Massaquoi and explained the purpose the visit. The current census is 6 with 2 facility staff present.
On 3/24/2026 the facility was issued (2) citations, due to the POCs not being corrected within the time-frame provided to the licensee during the previous visit. The following citations were reissued to the facility as these deficiencies pose a potential health and safety risk to residents in care.
The deficiency cited under Title 22 Regulation 87465(a)(6) has not been cleared. Licensee did not comply with the terms of the POC by POC due date.
The deficiency cited under Title 22 Regulation 87465(h)(5) has not been cleared. Licensee did not comply with the terms of the POC by POC due date.
As a result of this POC visit, the facility is not in compliance with Title 22 Regulations, and the deficiencies can be found on the LIC 809 D page. An exit interview was conducted over the phone with the licensee and a copy of these LIC 809 reports, LIC 809-D page, Appeals rights were provided to the facility.
