Facility Evaluation Report
On 05/20/26, Licensing Program Analyst (LPA) Kimberly Viarella made an unannounced visit to this facility to open an investigation into the above allegation. LPA identified herself upon arrival, stated the purpose of the visit, and asked to meet with the Designated Facility Administrator. LPA met with Maria Caldeira who contacted the Licensee, Ericson Serrano and the three discussed the completion of the closure plan for the facility. Caldeira stated that the missing components would be emailed to LPA Viarella after she and Serrano meet this evening. Caldeira then left the facility. Serrano remained to complete the visit.
LPA amended a report containing a deficiency dated 05/01/26 as it was incorrectly attached to an LIC 9099 and will be cited today on the LIC 809D during this case management.
LPA did a walkthrough of the facility. Checked the medication and food supplies for the remaining resident in care. Medication was in compliance, The food supply was lacking fresh fruits and vegetables aside from 1 bag of carrots. LPA provided technical assistance regarding appropriate foods for the remaining resident in care.
According to the California Code of Regulations, Title 22, no other citations were observed or cited during this visit. A copy of this report and APPEAL RIGHTS were provided and an exit interview was conducted with Serrano.
