Facility Evaluation Report
On 05/07/26, Licensing Program Analyst (LPA) Kimberly Viarella made an unannounced visit to this facility to conduct a case management visit to ensure the health and safety of the 2 remaining residents in care as this facility continues to proceed with its closure plan. LPA identified herself upon arrival, state the purpose of the visit and asked to meet with the Designated Facility Administrator, Maria Caldeira. Caldeira was not present at the facility. Staff called the Licensee, Ericson Serrano, and they arrived shortly after.
While waiting for the Serrano, LPA Viarella visited with resident R1, one of the 2 remaining residents in care. LPA observed a breakfast of pancakes, hard boiled eggs, cereal, fresh fruit, juice and tea. LPA also observed 1 pill box and an inhaler medication clearly labeled AM and PM. LPA took pictures for documentation purposes. LPA checked R1's Physician's report and all mediations were to be secured and administered by trained facility staff. This deficiency was cited on the LIC 809D page.
LPA observed the other resident in a recliner resting in the main TV room. All other residents have relocated to other care facilities. There was 1 care staff on duty. LPA inquired if R1 was a 2 person assist, Serrano said he believed so, but R1 "rarely like to get out of bed," when R1 needed/wanted to, there were carestaff on call and nearby if a two person assist was required.
LPA observed the food supply and the medications for the 2 remaining residents. Both were in compliance at the time of this inspection.
According to the California Code of Regulations, Title 22, no other deficiencies were cited during today's visit. A copy of this report was provided and an exit interview was conducted with Serrano.
