The inspector’s account

An informal conference was held virtually via Microsoft Teams on April 23, 2025. In attendance were Licensing Program Analyst (LPA) Vincent Moleski, Licensing Program Manager (LPM) Stephen Richardson, Alta California Regional Center (ACRC) representatives Jaspreet Mann, Christina Lane, Isabella Vega, Brenda Nguyen, and Wayson Yuen, and licensee Napoleon Mucha.

The purpose of this meeting was to discuss the 2016 termination of St. Philomena Home Care, LLC, the business entity which holds the license for this facility. After reviewing available Community Care Licensing Division (CCLD) records, LPM Richardson determined that, since the licensed entity was terminated, Mucha will have to reapply for a new license in order to continue operations. Since ACRC vendorization is tied to CCLD licensure status, Mucha will have to be re-vendorized once his new license is issued. LPM Richardson said this application process will be expedited in order to avoid resident relocations.

Both LPM Richardson and ACRC representatives emphasized that Mucha must act in a timely manner in order to secure a new license and new vendorization without disruption of services. Mucha agreed to turn in his application documents as soon as possible.

No deficiencies were cited during this meeting. An exit interview was held and a copy of this report was sent to Mucha to sign.
