Facility Evaluation Report
Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct an annual inspection. LPA Moleski met with program director Shobhna Mudaliar and explained the purpose of the visit.
LPA Moleski reviewed three client files (C1-C3) and three staff files (S1-S3). LPA Moleski reviewed Guardian records and observed that S1 has a criminal record clearance, but is not currently associated to this facility's staff roster. Mudaliar confirmed that S1 was hired in November 2024 and has been working daily since their hire date.
LPA Moleski toured the facility with Mudaliar and inspected common areas, kitchen areas, and classroom areas. Furniture and furnishings were sufficient to meet the needs of clients. The facility temperature was 70 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 106 degrees Fahrenheit, which is within the required range of 105 and 120 degrees.
LPA Moleski observed first aid supplies, a fully-charged and up-to-date fire extinguisher, and carbon monoxide/smoke detectors. LPA Moleski observed locked areas for the storage of cleaning solutions, knives, and medications.
LPA Moleski interviewed three staff members (S4-S6) and three clients (C3-C5).
This facility is hereby cited per 22 CCR Section 82019(e)(3). Due to a violation of criminal record clearance requirements, a civil penalty in the amount of $100 per day worked by S1, with a maximum assessed total of five days, is hereby assessed. An exit interview was held with Mudaliar. Appeal rights and a copy of this report was left with Mudaliar.
