Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct a case management visit in order to follow up on two incident reports. LPA Moleski met with facility administrator Orlando Carpio and explained the purpose of the visit.
An incident report received on 11/7/23 described a resident (R1) assaulting a staff member (S1) on 11/6/23. LPA Moleski interviewed Carpio.
No deficiencies were cited during this visit. An exit interview was held and a copy of this report was left with Carpio.
