Facility Evaluation Report
On 07/01/2026 Licensing Program Analyst (LPA) Shakaricka Hughes conducted an unannounced case management visit to the facility. The purpose of this visit was to follow up on recent incident reports received by the Department. LPA met with the facility administrator Orlando Carpio. The current census is 4 with 4 facility staff present.
On 06/22/2026 LPA Hughes received information that a resident would be issued a 30 day Eviction Notice due to recent incidents involving the resident (R1), other residents, and facility staff. During the visit, LPA spoke with the facility administrator Orlando, who stated that the new plans have been implemented coordinated with Regional Center to increased services to assist the resident. Administrator states that at this time the facility has not issued 30 day notice, and the residents will remain in the facility. Administrator stated that he will re-evaluate the plan currently in place for the resident in two months to determine whether the resident will require new placement.
