On 11-10-2021 Licensing Program Analyst (LPA) TIrzah Hubbard conducted an unannounced Post-Licensing visit. LPA Hubbard contacted Licensee Carpio to discuss reason for visit. The licensee was not available.
LPA Hubbard was not able to complete the post licensing visit due to the facilities annual being conducted to early by LPA Hubbard and LPA Yang. Based on review, in order to conduct a post-licensing visit the annual can not be completed first.
LPA Hubbard will return at a later time to conduct the Post-Licensing as a case management visit on LPM Richardson's approval.
