The inspector’s account

On 11/17/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct their annual inspection visit. LPA met with Administrator Okgi Min (AD), and stated the purpose of the visit.

Overview : Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 6 elderly residents. Facility is cleared to accept up to 6 non-ambulatory, 2 of which can be bedridden status. Facility is approved to retain/accept up to 3 residents receiving hospice services. Facility has fire sprinkler system installed.

Initial Observation: Upon arrival LPA observed AD with one staff on duty. All 6 residents were present. LPA observed required posters and facility license at the entrance. Room temperature was at 74 degrees Fahrenheit upon arrival.

Physical Inspection : Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA inspected 5 resident bedrooms and the 3 bathrooms. LPA measured the hot water temperature in the 1 of 2 bathrooms to be at 105 degrees Fahrenheit. Resident bathrooms were observed to be in clean and good repair at this time.

Fire extinguisher was observed in the kitchen area and was last inspected on 8/13/2025. Smoke and carbon monoxide detectors were observed throughout. LPA observed centrally stored medications, toxins, sharp objects and other dangerous items were kept locked and inaccessible to residents in care.
