The inspector’s account

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with facility staff Sharon Manalang, and explained the purpose of the visit.

LPA Valerio and staff toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed five (5) residents in care. All residents were in their bedrooms watching television, listening to music, or taking a nap. Resident rooms were free from odors, clean, and fully furnished. Resident bathrooms were observed to be fully stock with hygiene supplies, to deliver hot water that is within regulatory range, and to be in sanitary conditions. Common areas were organized and fully furnished. The kitchen was observed to have an adequate food supply. The exterior area was observed to an area for sitting under shade and two exit ways. Emergency exits were clear from obstructions. Sound alarms were observed to be in working condition. The fire extinguisher was last serviced on July 3rd, 2025. No health or safety concerns observed. Facility staff informed LPA that the licensee will pay the annual fees in the next upcoming month when it is due.

LPA Valerio reviewed two (2) staff files and two (2) resident files. LPA Valerio observed all files to be up to date with required documentation.

LPA Valerio requested the following: Copy of Updated Facility Sketch, Administrator Certificate/CEUs/ID, LIC 500, LIC 308, LIC 610, and Copy of liability insurance

No deficiencies were observed during today's visit. An exit interview was held, and a copy of the report was provided.
