Facility Evaluation Report
On 05/22/2026 Licensing Program Analyst (LPA) Shakaricka Hughes conducted an unannounced visit to the facility. LPA met with the facility administrator Monalie Pascual- Mercado. The current census is 4 with 3 facility staff present.
The purpose of this case management visit is to follow up on plan of corrections for cited deficiencies during a complaint visit related to complaint control # 27-AS-20260310105635. LPA Hughes cleared the plan of corrections (POC) for 1 deficiency cited: Title 22 regulation 80072(a)(3) Personal Rights.
During the visit LPA did not observe excluded individuals (E4)(E5(E6) providing care to residents in the facility. LPA checked Guardian rosters and did not observe the excluded individuals associated to the facility. LPA reviewed staff training documents for topics Recognizing Abuse, Reporting Requirements, and Mandated Reporting.
An exit interview was conducted a copy of this report and a cleared POC letter was provided to the facility.
