Facility Evaluation Report

Licensing Program Analyst (LPA) Christina Valerio arrived announced to conduct an annual required inspection. LPA Valerio met with Angelina Green, and explained the purpose of the visit.

LPA Valerio inspected the home and observed zero residents in care. The facility was observed to be move in ready. The facility bedrooms were observed to be fully furnished. Resident bathrooms were observed to be clean and sanitary. The fire extinguisher was fully charged and fire alarms were observed to be operational. No health or safety concerns were observed.

The facility is waiting to be vendorized with Alta California Regional Center.

Per California Code of Regulations (CCR) - Title 22 - no deficiencies were observed. An exit interview was held, and a copy of this report was provided.
