The inspector’s account

On 7/23/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct their annual inspection visit. LPA initially met with staff on duty Edna Villanueva (S2) and stated the purpose of the visit. The Administrator Lionel Nunez (S1) was notified and arrived shortly after.

Overview : Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 4 adult residents, ages 18 to 59, 3 ambulatory and 1 non-ambulatory. Facility is also vendored by Alta California Regional Center. Facility has 3 resident bedrooms and two resident bathrooms.

Initial Observation: Upon arrival LPA did not observe any residents. Per interview with S2, residents were out in the community. LPA observed required posters and facility license. Room temperature was at 76 degrees Fahrenheit upon arrival.

Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA observed the inside of the facility to be clean and in good repair at this time. LPA inspected 3 of 3 resident bedrooms and were observed to be equipped with the required furniture and sufficient lighting throughout. Bedroom #1 is cleared for non-ambulatory use. At this time, all residents in care are ambulatory. LPA measured the hot water temperature in the 1 of 2 bathrooms to be at 110 degrees Fahrenheit. Both resident bathrooms were observed to be in clean and good repair at this time.
