Facility Evaluation Report
On 7/31/24, Licensing Program Analyst (LPA) Arvin Villanueva arrived unannounced at this facility to conduct their required annual inspection. LPA met with staff on duty (S1) and stated the purpose of the visit. The administrator, Lionel Nunez, was notified of the visit. Lionel is unable to come to the facility at this time and gave permission to staff on duty to accommodate LPA and sign the report. Present during today's visit were 3 residents in care with 2 staff on duty.
LPA and S1 toured the facility physical plant to ensure compliance with Title 22 regulations. The facility is a one-story home located in a residential neighborhood. LPA observed the front yard to be clean and free of obstruction. LPA observed the backyard to have outdoor furniture. LPA did not observed any covered area for outdoor activities. LPA observed the fence at the right side of the facility to be in disrepair as evidenced by fence leaning and some parts of the fence not fully nailed/secured. Additionally, the gate was observed to be dragging at the bottom when LPA opened it which require force to pull the gate open.
LPA observed bedrooms to be furnished. 1 of 3 bedroom is a shared bedroom with its own bathroom. 2 of 3 bedrooms are single/private room. 1 of 2 single bedroom was unoccupied at this time. Bathrooms were fully stocked with toilet paper, paper towels, soap, trash can, and non-skid mats. In the master bath, the shower door was observed to get stuck and difficult to slide open and close. Common areas were observed to be clean and free from debris. LPA observed the temperature inside the facility was measured at 74*F and the hot water was measured at 108*F. Facility has nonperishable foods for a minimum of one week and fresh perishable foods for a minimum of two days. LPA observed the centrally stored medications area, toxins, and sharps to be locked and inaccessible to clients. LPA observed a pull alarm system, fire extinguisher(s) with last serviced on 05/17/24, smoke and carbon monoxide detectors, central heating and air in the facility to be in good working condition. Fireplace was observed to be partially screened and accessible to residents in care. Per interview with S1, fireplace has never been utilized. No emergency exits were obstructed.
