Facility Evaluation Report

Licensing Program Analyst Arvin Villanueva (LPA) arrived unannounced to conduct a continued annual Inspection on today's date of 4/29/2025 at 9:17am. Initially, there was no one at the home. LPA notified the designated staff Kathy Ryhmes and informed her of this visit. Kathy informed LPA that she will send a staff to assist with this visit. Later at 9:58am, staff Maria Salud Galvez arrived and LPA stated the purpose of this visit.

Upon arrival, all clients in care were out in the community. Later during this visit, 2 clients in care arrived from their appointments with 2 staff.

Prior to this visit, Kathy Rhymes reported that the gate that was observed to be screwed shut is permanently closed and will be submitting a new facility sketch to the Department. The facility has one exit gate - located next to the bigger garage.

During this visit, LPA conducted 2 client record reviews and found to be in compliance at this time. LPA reviewed 2 client medications and found to be in compliance at this time. LPA also reviewed 2 client cash resources record with staff Maria Salud Galvez and found to be accurate at this time. LPA conducted 4 staff record reviews and found to be in compliance at this time. Facility conducts monthly drills and last drill was conducted on 4/17/25.

LPA requested updated copy of LIC 308, LIC 500, surety bond, updated facility sketch and liability insurance to be submitted by 5/2/2025.

The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22. Failure to correct deficiencies may result in civil penalties.

An exit interview was conducted with Maria Saludgalvez and a copy of this report and appeal rights were provided.
