The inspector’s account

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with facility staff Kyaira Cordoba, and explained the purpose of the visit. LPA was later met by Administrator Karen.

LPA Valerio toured the facility to ensure compliance of Title 22 regulations. LPA Valerio observed the common areas to be clean and free from debris. Resident bedrooms were fully furnished, free from odors, and personalized. Residents bathrooms were observed to be clean and fully stocked. Medications, sharps, and toxins were observed to be locked and inaccessible to resident in care. Hot water was measured between the regulatory range 105-120 degree F. The kitchen was observed to be clean. The facility was observed to have an adequate supply of food. The fire extinguisher was observed to be fully charged and in working condition. The date of inspection was serviced on April 28, 2025. The backyard was observed to have an area for outside activities and exercises. The shed was being utilized for storage. No emergency exits were obstructed. LPA Valerio observed staff assisting residents with ADLs, snacks, and activities.

LPA Valerio reviewed staff and resident files, all of which were up to date with required documentation. LPA Valerio requested the following be sent to LPA Valerio: LIC 500, LIC 308, LIC 610, Copy of Surety Bond

Per California Code of Regulations (CCR) - Title 22 - no deficiencies were observed. An exit interview was held, and a copy of this report was provided.
