Facility Evaluation Report

On 12/5/2025, Licensing Program Analyst, Arvin Villanueva (LPA), arrived at this facility to conduct the annual inspection visit. LPA initially met with staff on duty, Sekina Khairedine and stated the purpose of the visit. The Licensee/Administrator, Fozia Hassen, arrived shortly after.

Overview : Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 4 adult residents, 3 of which may be non-ambulatory. Bedroom #3 is approved for ambulatory residents only. Facility does manage residents’ cash resources. Facility is also vendored by Alta California Regional Center (ACRC).

Initial Observation: Present were 2 residents with 1 staff on duty. One resident is currently out in the community. LPA observed required posters and facility license. Room temperature was at 74 degrees Fahrenheit upon arrival.

Physical Inspection : Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA inspected 6 of 6 resident bedrooms and 2 of 3 bathrooms. The hot water temperature was measured at 108 degrees Fahrenheit. Advisory was provided to Fozia to continue to monitor the hot water temperature. Per review of their hot water documentation, temperature fluctuate but mostly within regulatory standard. Resident bathrooms were observed to be clean and in good repair at this time.

Fire extinguisher was observed in the kitchen area and was last inspected on 6/12/2025. Smoke and carbon monoxide detectors were observed throughout. LPA observed centrally stored medications, toxins, sharp objects and other dangerous items were kept locked and inaccessible to residents in care.
