Facility Evaluation Report

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio was met by facility staff Michelle, and explained the purpose of the visit. LPA was later met by Administrator Jhoner Palor.

LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed the facility common areas (kitchen, dinning room, living room, and exterior space) to be clean, organized, fully furnished, and free from odors. All areas were observed to be fully furnish with plenty of room for activities and lounging. The facility was observed to have an adequate supply of nonperishable and perishable food items. Resident rooms were observed to be clean, fully furnished, personalized to each individual, and free from odors. Resident bathrooms were clean and stocked with hygiene supplies. Hot water temperature was observed to be within the regulatory range of 105.0-120.0 degrees. LPA Valerio observed medications, sharps, and hazardous materials to be locked and inaccessible to residents in care. LPA Valerio observed the garage, supply closets, staff room, and exterior areas. No health, safety, or personal rights concerns observed.

LPA Valerio reviewed two (2) staff files and two (2) resident files. All files were observed to be up to date.

LPA Valerio requested the following annual documentation: LIC 500, LIC 308, LIC 610, and copy of surety bond

Per California Code of Regulations (CCR) - Title 22, Division 6, Chapter 6, no deficiencies were observed during this visit. An exit interview was held with Administrator Jhoner Palor, and a copy of the report was provided to him.
