Facility Evaluation Report
On 05/14/2026, Licensing Program Analyst (LPA) Pang Lee arrived at the facility to conduct an unannounced annual inspection. Upon arrival, LPA Lee rang the doorbell and knocked on the door; however, no one answered. LPA Lee then contacted Facility Designated Administrator Hugh Le by telephone and informed him that Community Care Licensing Division (CCLD) staff were present to conduct the facility’s annual inspection. Approximately 20 minutes later, Administrator Le arrived at the facility and assisted with the visit. Administrator Certificate # 7019742735 expires on 12/05/2027. The current census was two (2) clients in care.
This facility is a single-story building licensed to serve four (4) clients, including three (3) ambulatory clients and one (1) non-ambulatory client located in bedroom #4. LPA Lee inspected the physical plant including but not limited to the common area, kitchen, dining area, client bedrooms, client bathrooms, laundry room, garage, staff office and outside courtyards of the facility to ensure compliance with Title 22 regulations. LPA Lee observed the facility to be free of odor, clean and in good repair. LPA Lee observed bedrooms to be properly furnished with appropriate bedding and lighting. There are no bodies of water present. LPA Lee toured the kitchen and observed sufficient seven-day non-perishable and two-day perishable food supplies. Hot water temperature was measured at 116.1 degrees Fahrenheit in client bathroom sink, which is within the required regulation of 105 to 120 degrees Fahrenheit. The smoke and carbon monoxide detectors are in compliance with fire safety. The fire extinguisher is located in the hallway of the facility and was last serviced on 02/02/2026. The last fire drill was conducted on 04/09/2026.
