Facility Evaluation Report
On June 8 2026, Licensing Program Analyst, Arvin Villanueva (LPA) arrived to this facility unannounced to conduct their quarterly case management visit due to a current stipulation order in place. The facility is licensed under a probationary license.. LPA met with the facility administrator, Betty Jeanne Fillon and Roselyn Fillon, and stated the purpose of the visit. During this visit, residents in care were out in the community. .
Per stipulation, licensee was granted probationary license subject to the following limitations and conditions:
Posting stipulation was observed to be posted in conspicuous location. LPA observed posted at the hallway.
Documentation: Current client roster is maintained. LPA reviewed 4 staff files and all have current CPR/First Aid certification on file. Per review of the Personnel Roster (LIC500), all individuals currently working at this facility have current criminal records clearances. Per record review, all 4 residents do not have restricted health condition at this time.
Training Compliance: Licensee completed the first training within the 30 day of the date of the stipulation. Reminder provided that ongoing training needs to be updated every 6 months and that proof of these training shall be maintained at this facility and be available for review at any time.
Facility Operations and Conditions: LPA conducted a physical inspection of the facility, both interior and exterior. Per observation, facility is clean, safe, sanitary and in good repair at this time. Facility maintains adequate food supply. All cleaning supplies, sharp objects, medications and other similar items were kept locked. Hot water temperature was at 106.3 degrees Fahrenheit. Advisory was provided to obtain a new thermometer to use to monitor their hot water temperature.
Based on today's visit, no deficiencies are being cited. Exit interview was conducted and a copy of this report was provided.
