The inspector’s account

Licensing Program Analyst (LPA) Christina Valerio arrived to the facility unannounced to conduct an annual required inspection. LPA Valerio met with facility staff Rocelyn Bernabie-Fillon and Administrator Betty Jeanne Filon, and explained the purpose of the visit.

LPA Valerio observed residents coming back from day program. LPA Valerio observed four residents in care. Facility staff were observed assisting residents with ADLs, preparing food, and providing snacks to the residents. Residents were observed doing at home activities, sitting in their personal chair with blankets, and eating their snacks. LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed residents rooms to clean and free from odors. Resident bathrooms were clean, sanitary, and fully stocked with hygiene supplies. Common areas were fully furnished, clean, and free from odors. Medications, sharps, and toxins were inaccessible to residents in care. The facility was observed to have an adequate food supply. The fire extingusher was fully charged with the last annual check on 04/2025.

LPA Valerio observed two (2) resident files and two (2) staff files. Files were observed to be complete with up annual documentation. LPA Valerio requested the following annual documentation be sent to LPA Valerio email: LIC 500, LIC 308, LIC 610, Copy of surety bond, and copy of liability insurance

Per California Code of Regulations (CCR) - Title 22, no deficiencies were cited today. An exit interview was held, and a copy of this report was provided.
