The inspector’s account

Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct an annual inspection. LPA Moleski met with facility administrator Bayarmaa Ragchaa and explained the purpose of the visit.

LPA Moleski reviewed four resident files (R1-R4) and three staff files (S1-S3). LPA Moleski reviewed an LIC 500 for this facility, current as of today's date, 1/30/25. The LIC 500 listed two staff members (S2 and S3) as live-in staff on the weekends. LPA Moleski reviewed this facility's sketch, and observed that there are no staff bedrooms present in the facility, and the only bedrooms present are designated as private resident bedrooms. LPA Moleski asked Ragchaa and S1 where the facility's live-in staff sleep on the weekends. Ragchaa and S1 both said that the live-in staff sleep on separate couches, one located in the facility's living room, and the other located in a common area adjacent to the main entryway. Ragchaa said that this arrangement has been in place since S2 and S3 started working at this facility in September 2024.

LPA Moleski toured the facility with Ragchaa and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas. Furniture and furnishings were sufficient to meet the needs of residents. The facility temperature was 72 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 123 degrees Fahrenheit, which is not within the required range of 105 and 120 degrees.

LPA Moleski observed first aid supplies, a fully-charged and up-to-date fire extinguisher, and working carbon monoxide/smoke detectors. LPA Moleski observed a minimum 2-day supply of perishable food and a minimum 7-day supply of nonperishable food. LPA Moleski observed a locked closet for the storage of medication. LPA Moleski observed locked cabinets for the storage of cleaning solutions and knives. LPA Moleski interviewed one staff member (S1) and one resident (R1). This facility is hereby cited per 22 CCR Sections 85087(a)(3) and 80088(e)(1). An exit interview was held with Ragchaa. Appeal rights and a copy of this report were left with Ragchaa.
