The inspector’s account

On 7/3/2026 Licensing Program Analysts (LPAs) Reza Jamaly and Christina Valerio made an attempted visit to the facility to conduct 1-year required inspection visit. LPA noticed no one is at the facility, office door was closed, parking lot was empty. LPA Christina placed a call to the facility number on file, it was not answered.

On 7/9/2026, at 9:20 am, Licensing Program Analysts (LPAs) Reza Jamaly and Christina Valerio arrived unannounced at the facility to conduct the required annual inspection. LPAs met with Program Directors (PD) Augusta Osayande and Garrett Keisler, and stated the purpose of the visit. The PDs accompanied LPAs during the facility inspection.

LPAs toured all five classrooms, the kitchens, bathrooms laundry room, craft cabinets, and the outdoor areas.

Classroom 5: Participants were actively engaged in games and other scheduled activities under staff supervision. LPAs observed labeled lunch boxes stored in the classroom. The Program Director stated that the program does not routinely provide meals, and participants bring their own lunches except during special occasions, such as birthdays and other celebrations.

LPAs inspected the kitchen, including multiple cabinets, refrigerators, and freezers. Sharp objects were stored in locked cabinets and were inaccessible to participants. Refrigerators and freezers containing participants and staff snacks and were clean and organized, and staff food items were appropriately labeled.
