Facility Evaluation Report
On 05/01/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA Pascua was greeted by Site Coordinator, Brandy Arismende and explained the purpose of the visit.
LPA Pascua asked SD to call the Program Director,Augusta Osayande to inform them that CCL was present at this time. Shortly after, LPA Pascua met with Program Director and Facility Designated Administrator, Joe Jaquez.
Current census was 61. A brief interview with Program Director (PD) Augusta Osayande was conducted.
This facility is licensed to serve 90 clients, of which 30 may be non-ambulatory. This facility currently has 56 clients on site with 32 staff members.
LPA Pascua reviewed 4 resident files and 4 staff files. All files were complete and up to date.
A tour of the facility was conducted. This facility has 4 buildings on site.
Dining area, kitchen area, and all other areas designated for client use were observed to be furnished and maintained in compliance at this time.
All areas designated as classrooms, activity areas, and common areas for client use were toured. Furniture and furnishings were observed to be present and sufficient to meet the needs of the clients at this time.
Office rooms and other areas intended for client use were toured.
A review of the day program restrooms was conducted at this time.
Hot water temperatures were taken to make sure that the hot water being dispensed was within the allowed range of 105-120 degrees at all times.
Fire extinguishers, located throughout this facility, were observed to have been annually inspected on 04/30/2026, and in compliance at this time.
