On 12/12/25, Licensing Program Analyst (LPA) Cynthia Tamayo made an unannounced visit to this facility to conduct a collateral visit in reference to a unrelated to the functions at the facility (# 347001442- ELK GROVE ADULT COMMUNITY TRAINING,INC. ACT II, located at 9510 ELK GROVE FLORIN RD.,#C). LTC Ombudsman (P1), and Alta Regional Service Coordinator (P2) were present during this visit.
According to the California Code of Regulations, Title 22, there were no deficiencies observed or cited during today's visit. A copy of this report was provided and an exit interview was conducted with Brandy Arismende (P3).
