Facility Evaluation Report
Licensing Program Analyst (LPA) Renee Campbell arrived unannounced to conduct an annual inspection. LPA Campbell met with Mary Ann De Los Santos, Licensee and explained the purpose of the visit. During the visit, LPA Campbell observed residents engaged in activities with other residents and staff.
The facility is a one story Residential Care Facility for the Elderly licensed (#342700772) to service adults over the age of 60. Six of the residents may be non-ambulatory and there is a hospice waiver for three residents. There are three bathrooms and five bedrooms within the facility.
LPA Campbell toured the facility and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents. Furniture and furnishings were sufficient to meet the needs of residents. The facility temperature was 72 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature was measured at 107 degrees Fahrenheit (F), which is within the required range of 105 and 120 degrees. The refrigerator temperature was set at 43 degrees F. The freezer was a -4 degrees F.
