The inspector’s account

On 2/10/2026, Licensing Program Analyst, Arvin Villanueva (LPA), arrived unannounced to conduct the annual inspection visit. LPA initially met with staff on duty (S1) and stated the purpose of the visit. The Licensee/Administrator, Rowena Villanueva (AD), arrived shortly after. Another staff member (S2) also arrived.

Overview : Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 4 adult residents. Bedroom #3 is approved for non-ambulatory use. Facility does manage residents’ cash resources. Facility is also vendored by Alta California Regional Center (ACRC).

Initial Observation: Present were 2 residents with 1 staff on duty. One resident is currently out in the community. LPA observed required posters and facility license. Room temperature was at 74 degrees Fahrenheit upon arrival.

Physical Inspection : Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA inspected 4 of 4 resident bedrooms and 2 bathrooms. The hot water temperature was measured at 112 degrees Fahrenheit. Room temperature was maintained at 68 degrees Fahrenheit.

Fire extinguisher was observed in the dining area and was last inspected on 9/30/2025. Smoke detectors were observed throughout. LPA observed at least one carbon monoxide monitor. LPA observed centrally stored medications, toxins, sharp objects and other dangerous items were kept locked and inaccessible to residents in care.
