Facility Evaluation Report

Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with Administrator Cynthia Cruz-Maduro, and explained the purpose of the visit.

LPA Valerio observed staff at the facility. The resident was out of the facility on an outing during the visit. LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed common areas (living room, dinning room, kitchen, hallways, and exterior area) to be fully furnished, organized, and free from debris. Resident bedrooms were observed to fully furnished and clean. Resident bathroom was observed to be fully stocked with hygiene supplies. The facility was equipped with an emergency supply of food, a first aid kit, and extra linens/supplies. The kitchen was observed to have an adequate food supply. Hot water temperature was measured within the regulatory range of 105.0-120.0 degrees. No emergency exits were obstructed.

According the administrator, they are working with their local contractor to complete the POC for the visit conducted by LPA Campbell on 09/25/2025. Administrator to update LPA Valerio on POC when complete.

LPA Valerio observed one (1) resident file and staff files. Resident and staff files were observed to complete with required documentation. LPA Valerio requested the following: Updated facility sketch (when completed), LIC 500, LIC 308, LIC 610, surety bond, and liability insurance.

Per California Code of Regulations (CCR) - Title 22 - no deficiencies are being cited today. An exit interview was held with administrator, and a copy of the report was provided.
