Facility Evaluation Report
Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct an annual inspection. LPA Moleski met with facility administrator Meggin Cortez and explained the purpose of the visit.
LPA Moleski reviewed five resident files (R1-R5) and five staff files (S1-S5).
LPA Moleski toured the facility with Cortez and inspected common areas, kitchen areas, resident bedrooms, bathrooms, and outdoor areas. The facility temperature was 76 in memory care and 80 degrees in assisted living, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 116 in memory care and 115 in assisted living, which is within the required range of 105 and 120 degrees.
LPA Moleski observed first aid supplies, fully-charged and up-to-date fire extinguishers, and carbon monoxide/smoke detectors. LPA Moleski observed a minimum 2-day supply of perishable food and a minimum 7-day supply of nonperishable food. LPA Moleski observed a locking medication room for the storage of medication. LPA Moleski observed locked storage closets for the storage of cleaning solutions.
LPA Moleski interviewed four staff members (S1, S6-S8) and four residents (R5-R8).
No deficiencies were cited during this visit. An exit interview was conducted and a copy of this report was left with Cortez.
