Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct a case management visit. LPA Valerio met with facility Alex Del Carmen, and explained the purpose of the visit.
LPA Valerio confirmed with facility staff that Staff 1 (S1) no longer works for the organization. LPA Valerio confirmed that the staff working were not S1.
LPA Valerio ensured that S1 was separated from their Guardian Roster.
No health or safety concerns were observed.
An exit interview was held, and a copy of this report was provided.
