Facility Evaluation Report
Licensing Program Analyst (LPA) Renee Campbell arrived at the facility to conduct an unannounced annual inspection on 07/21/2025. LPA met with Florinda Fuentes, Administrator and explained the purpose of the visit.
There were four residents living in the community. Upon entry, two (2) residents were observed preparing to leave for day program. One resident was getting his face shaved and another resident was sitting on the couch. All residents had eaten breakfast for the day.
LPA Campbell inspected the physical plant including but not limited to the common area, kitchen, dining area, client bedrooms, client bathrooms, laundry room and outside courtyards of the facility to ensure compliance with Title 22 regulations . This facility is a single story building licensed to serve three (3) ambulatory and one (1) non-ambulatory resident. LPA Campbell observed the facility to be free of odor, clean and in good repair. LPAs observed bedrooms to be properly furnished with appropriate bedding and lighting. There are no bodies of water present .
LPA Campbell observed sufficient seven-day non-perishable and two-day perishable food supplies. Hot water temperature was measured in the kitchen and bathroom. The kitchen water temperature measured at 113 degrees Fahrenheit (F) and the resident bathroom water temperature measured at 114 degrees F. Fire extinguishers, smoke and carbon monoxide detectors are in good repair. The fire extinguisher was last inspected on 05/19/2025 and the smoke/carbon monoxide alarms were tested and found to be functioning. Facility thermostat observed at 70 degrees Fahrenheit. LPA Campbell checked medication storage and found medication to be locked away and inaccessible to clients. First aid kit was complete and contained scissors, tweezers and bandages. A therrmometer was observed in the community as well; LPA requested staff files for review. LPA reviewed 3 staff files of 22 staff. Toxins were made inaccessible to clients in care. Toxins were stored in a storage closet near the bedrooms and was observed to be locked.
