The inspector’s account

An informal conference was held virtually via Microsoft Teams on April 18, 2025. In attendance were Licensing Program Analyst (LPA) Vincent Moleski, Licensing Program Manager (LPM) Stephen Richardson, licensees Rochelle Cister and Verne Cister, and their daughter, Veronica Cister.

The purpose of this meeting is to discuss recent organizational changes in the LLC which is the licensee of this facility, and another facility owned/operated by Rochelle and Verne Cister. The initial LIC 309 for this facility, dated 3/1/22, indicated that the licensee, Cisters Loving Care Home LLC, was owned by Rochelle Cister and Verne Cister and had no additional managing members. An amendment filed with the California Secretary of State shows that two new members -- Veronica Cister and another family member -- were added to the LLC as managing members as of 6/3/24. An updated LIC 309 for this facility dated 4/4/25 shows updated ownership percentages of 40% for Rochelle Cister, 40% for Verne Cister, and 20% between both of their daughters. Because this constitutes a minority change of ownership, no additional action is required by the Community Care Licensing Division (CCLD) at this time. However, LPM Richardson requested that Rochelle Cister reach out to Alta California Regional Center to see if they may require any additional steps due to this organizational change. LPM Richardson also discussed two recent complaint investigations, the allegations of which were determined to be unsubstantiated as of 10/10/24 and 12/6/24 respectively. Although all allegations were unsubstantiated, LPM Richardson inquired into steps taken to avoid future incidents. Rochelle Cister said that her staff were retrained on abuse prevention, mandated reporting requirements, behavior intervention plans, and clients' rights. LPM Richardson requested documentation of this training. Rochelle Cister agreed to provide said documentation. This may be sent to LPA Moleski for review. Rochelle Cister also agreed to receive a referral to the Technical Support Program. No deficiencies were cited during this meeting. An exit interview was held and a copy of this report was emailed to Rochelle Cister to sign.
