On 09/18/2025 at 11:15 AM, Licensing Program Analyst (LPA) Shakaricka Hughes conducted an unannounced Case Management visit to serve the facility an Immediate Exclusion Order for staff member R1.
LPA met with facility Licensee Cherry Madamba and provided the Immediate Exclusion Order for R1. LPA Hughes requested that the Administrator disassociate R1 from the facility in Guardian and be removed from all shifts. Per administrator, R1 is not scheduled to work in the facility on 9/18/2025.
Exit interview conducted and a copy of this report was provided to the facility.
