Facility Evaluation Report
On 07/31/2025 at 3:45 pm, Licensing Program Analyst (LPA) Renee Campbell conducted an unannounced Case Management visit to serve the facility an Immediate Exclusion Order for staff member R1.
LPA met with facility Licensee Cherry Madamba and provided the Immediate Exclusion Order for R1. Statements from the facility administrator indicate the individual is no longer working at this facility and LPA Campbell requested that the Administrator disassociate R1 from the facility. During the visit, the Administrator submitted an email request to disassociate R1 from their facility.
Exit interview conducted and a copy of this report was left at the facility.
