Facility Evaluation Report
On 06/24/2026, Licensing Program Analyst (LPA) arrived unannounced to this facility to conduct an annual visit. LPA Pascua met with Facility Designated Administrator (FDA), Ada Togharanrose Omatseye and explained the purpose of the visit.
It was learned that this facility has been re-vendorized by Alta Regional Center to accept and retain Level 4 residents at this time. In addition, this facility is licensed to have 4 ambulatory residents.
There were currently no residents in care. The FDA Omatseye has an active administrator certificate #7038300735 and expires on 10/29/2026.
A tour of the facility was conducted to ensure that facility safety was in compliance, such as the fire extinguishers, smoke detectors, and carbon monoxide were present. All were present and in good repair at this time. A tour of facility bedrooms were conducted to verify that there are currently no residents in care.
LPA advised FDA Omatseye to notify the department once the facility obtains their first resident.
Based on the observations made, there are no deficiencies observed during this annual visit.
An exit interview was conducted and a copy of this report was provided to the facility.
