Facility Evaluation Report
LPA Sommer Hayes met virtually with Licensee Togharanrose “Ada” Omatseye and Designated Facility Administrator Chinenye “Chi” Obiago for a case management visit. The purpose of the meeting was to discuss whether they wish to continue maintaining their facility’s license with Community Care Licensing Division (CCLD) following the termination of vendorization through Alta California Regional Center (ACRC) as stated in the December 8, 2025 letter.
During the meeting, Ada and Chi asked about alternative ways to obtain clients for their facility, such as private pay or other insurance options, as they are currently evaluating whether to retain their license. They will explore these options further and decide at a later date. They also informed LPA Hayes that their last Alta Regional client (R1) will be discharged from the facility on or before January 7, 2026.
Ada and Chi stated their intention to appeal the vendorization termination and indicated that, if successful, they could reapply for vendorization. LPA Hayes scheduled a follow-up virtual meeting with Ada and Chi for early January 2026 to receive updates. They also inquired about the possibility of a Change of Ownership (CHOW), and a follow-up virtual meeting specifically for this and other updates were scheduled for January 12, 2026. LPA Hayes also shared that we do not offer resources for other referral agencies.
