The inspector’s account

Licensing Program Analysts (LPAs) Vincent Moleski and Jason Lund arrived unannounced on Monday, April 24, 2023 to conduct a case management visit regarding the immediate exclusion of an employee. LPAs Moleski and Lund met with administrator Jennell Revera and explained the purpose of the visit.

LPAs Moleski and Lund inquired regarding the status of S1. Revera said S1 was terminated on Saturday, April 22, 2023, immediately after receiving the immediate exclusion letter.

No deficiencies were cited during this visit. A copy of this report was left with Revera.
