Facility Evaluation Report
Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct a case management visit. LPA Moleski met with administrator Anne Carillo and explained the purpose of the visit.
LPA Moleski provided Anne Carillo with a copy of this facility's updated license. Cris Carillo, the designated facility administrator, had previously requested to have this facility's capacity reduced to four. This updated license, signed by LPA Moleski, reflects this new capacity. LPA Moleski retrieved the facility's previous license after providing Anne Carillo with the new license.
No deficiencies were cited during this visit. An exit interview was held and a copy of this report was left with Anne Carillo.
