Facility Evaluation Report
Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct a case management visit. LPA Moleski met with facility administrator Cris Carillo and explained the purpose of the visit.
LPA Moleski had previously visited this facility on 6/16/25 regarding the death of a resident (R1) which had occurred on 6/14/25. During that visit, LPA Moleski interviewed Carillo and a staff member (S1), who both said that although R1 was suffering from a cough in the days before R1's death, there were no signs of significant illness. Based on interviews and record review, R1 had attended a video appointment with their doctor on 6/10/25 and had been taking PRN medication to address the cough.
The Community Care Licensing Division (CCLD) reviewed a death certificate provided by county officials. The death certificate indicated R1's primary diagnosis was the immediate cause of death. No additional contributing factors were identified.
No deficiencies were cited during this visit. An exit interview was held and a copy of this report was left with Carillo.
