Facility Evaluation Report

Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to conduct a case management visit. LPA Moleski met with facility administrator Cris Carillo and explained the purpose of the visit.

Carillo emailed LPA Moleski an incident report on 6/14/25 regarding the death of a resident (R1). According to the incident report, R1 was found unresponsive by staff during a routine check around 4:15 a.m. on 6/14/25. Staff called 911 and emergency responders attempted CPR, but eventually declared R1 dead, according to the incident report. R1 was 71 years old.

During this visit, LPA Moleski interviewed Carillo and a staff member (S1), and reviewed R1's file.

No deficiencies were cited during this visit. An exit interview was held and a copy of this report was left with Carillo.
