Facility Evaluation Report

On 8/27/2025, Licensing Program Analyst, Arvin Villanueva (LPA) arrived unannounced at this facility to conduct the annual inspection visit. LPA initially met with staff on duty (S1) and stated the purpose of the visit. The Administrator, Roberto Gellegani (S2), was notified and arrived shortly after.

Overview : Facility is a one-story home located in a residential neighborhood. Facility is licensed to serve up to 4 adult residents, ages 18 to 59, all ambulatory only. Facility is also vendored by Alta California Regional Center. Facility has 4 resident bedrooms and two resident bathrooms.

Initial Observation: Upon arrival LPA observed the front yard is under construction. Per interview with staff, the pipes are being replaced due to frequent toilet clogs. Present during this visit were 2 residents in care with one staff on duty (S1). Residents were in their bedrooms during this visit.

Areas inspected include, but not limited to, the kitchen, resident bedrooms, resident bathrooms, living and dining room and outdoor areas. LPA inspected 2 of 4 resident bedrooms and were observed to be equipped with the required furniture and sufficient lighting throughout. 2 of the 4 bedrooms are currently vacant. LPA measured the hot water temperature in the 1 of 2 bathrooms to be at 106 degrees Fahrenheit. Room temperature was at 73 degrees Fahrenheit.

Fire extinguisher was observed in the dining area and was last inspected on 7/21/2025. Smoke and carbon monoxide detectors were observed throughout, tested and found operable at this time. LPA observed centrally stored medications, toxins, and sharp objects were kept locked and inaccessible to residents in care.
