The inspector’s account

On 1/23/25, Licensing Program Analyst (LPA) Arvin Villanueva arrived at this facility unannounced to conduct a case management visit. LPA met with facility administrator, Roberto Gellegani, and stated the purpose of the visit. Present during today's visits were 2 clients in care with 3 staff on duty.

The Regional Office received a Facility Action Report (FAR) dated 12/18/2024 from Alta California Regional Center (ACRC). ACRC representative conducted an inspection visit to the facility and identified several deficiencies. According to the FAR, the following issues were noted during the visit: one staff member (S1) lacked the minimum six months of prior experience providing direct supervision and special services and was required by Title 17 to acquire additional 12 hours of continuing education within 6 months of employment. It was noted that S1 did not complete their required hours of continuing education.

During this visit, LPA reviewed staff file for S1, including their health screening, first aid certification, and other training completed since their employment. The record review showed that prior to their employment date of 5/23/24, S1 completed 14 hours of Professional Assault Crisis Training (ProAct) on 5/18/24. Additional records indicated that S1 also received training in Facility Program Design, Individual Program Plan, Client's Rights Regulation, Medication Assistance, Health and Emergency Procedures, Special Incident Reporting, and Client Abuse Identification Reporting on 5/19/24. S1 is currently associated with this facility and completed their First Aid and CPR training on 6/6/2024.

Prior to today's visit, the Administrator has addressed and completed the necessary corrections as stipulated by ACRC. Advisory was provided to administrator to ensure they comply with all applicable laws and regulations.

Based on today's inspection, per the California Code of Regulations, Title 22, Division 6, Chapter 6, no deficiencies were cited.
