Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived to the facility unannounced to conduct an annual required inspection. LPA Valerio was met by Administrator Anne Carillo, and explained the purpose of the visit.
LPA Valerio observed three (3) staff on shift. Staff were observed conducting administrative task, cleaning, and doing daily house chores. LPA Valerio observed the common areas to be fully furnished, clean, and free from odors. The kitchen was sanitary and observed to have sharps/medications locked and inaccessible to residents in care. The facility was observed to have an adequate food supply to meet Title 22 requirements. LPA Valerio observed resident bedrooms, which were observed to be full furnished, personalized to each resident, and free from odors. Items in bedrooms were good repair. Resident bathrooms were observed to be clean and fully stocked with hygiene supplies. The facility was observed to have extra linens and towels for resident use. The temperature inside the facility was set to a comfortable temperature. The hot water was measured in the bathroom and kitchen, which was observed to be within 105-120 degrees F. Facility fire extinguisher was observed to be fully charged and operational. The last fire drill was conducted on 12/2025.
LPA Valerio observed three (3) resident files and three (3) staff files. All files were observed to have annual required documentation with up to date documents. LPA Valerio requested the following be sent to LPA's email: LIC 500, LIC 308, LIC 309, LIC 402, LIC 610E, and copy of liability insurance
No deficiencies were observed during today's visit. An exit interview was held with Administrator Anne Carillo, and a copy of this report was provided.
