The inspector’s account

Licensing Program Analyst (LPA) Vincent Moleski arrived unannounced to follow up on a complaint investigation, but discovered an unrelated deficiency during that visit. LPA Moleski spoke with facility administrator Beatrice Clark over the phone and explained the purpose of the visit.

LPA Sommer Hayes visited this facility on 10/16/25. During that visit, LPA Hayes requested several documents, including a resident roster, a staff roster, and a certain resident's (R1's) medical assessment and service plan. LPA Hayes requested these documents by 10/20/25. As of today, LPA Hayes has not yet received these documents. During this visit, LPA Moleski requested R1's medical assessment and service plan to assist in investigating the aforementioned complaint. However, Clark told LPA Moleski that she did not have immediate access to the documents, and she was planning on sending them to LPA Hayes later today.

22 CCR Section 87755(c) establishes inspection authority as follows: " (c) The licensing agency shall have the authority to inspect, audit, and copy resident or facility records upon demand during normal business hours. "

This facility is hereby cited per 22 CCR Section 87755(b) . An exit interview was held with Clark. Appeal rights and a copy of this report was left with staff member Leashia Tyrell-Moore.
