Facility Evaluation Report
Licensing Program Analyst (LPA) Christina Valerio arrived unannounced to conduct an annual required inspection. LPA Valerio met with facility staff Corazon Simpauco, and explained the purpose of the visit. LPA Valerio was later met by Licensee Rose and Administrator Albert Lopez.
LPA Valerio toured the facility to ensure compliance with Title 22 regulations. LPA Valerio observed two staff on shift and residents in different areas of the home. Staff were cleaning, doing administrative tasks, and assisting residents with ADLs. Residents were observed in their room, walking in the backyard, or enjoying snacks. Medications, sharps, and toxins were locked and inaccessible to residents in care. Resident bedrooms were clean and free from odors. Resident bathrooms were fully stocked with hygiene supplies, skid mat, hand rails, and a trash can. Hot water was measured at 118.2 degrees F. Common areas were observed to be fully furnished and organized. The facility temperature was at a comfortable temperature for residents in care. Fire extinguisher was observed to be fully charged with a last inspection date of May 18, 2025. The last fire drill was held on November 22, 2025. No emergency exits were obstructed.
LPA Valerio reviewed three staff files and three resident files. All files were observed to be up to date.
LPA Valerio requested the following documentation: LIC 500, LIC 308, LIC 610D, Liability Insurance
Per California Code of Regulations (CCR) - Title 22, no deficiencies are being cited. An exit interview was held with license/Administrator, and a copy of this report was provided.
